Family Assistance Center

The Family Assistance Center (FAC) remains open and will be available in coordination with our local community partners. Once the Center is closed, a telephone number will be available to residents seeking additional information and assistance.

Claims forms are available in person at the FAC. Residents may bring the necessary forms to the FAC, or complete them when they arrive. Please review the items listed below to be sure you bring all supporting materials that must be submitted along with your claim.

The center is relocating to a permanent location in East Palestine, serving both residents and businesses.

The center will be open:

Monday–Friday: 10 a.m. – 6:30 p.m.

Saturday: 10 a.m. – 4 p.m.
Sunday: Closed

Closed for Memorial Day Weekend, beginning Friday, May 26.

Hours available while
there is community need.

What should I bring?

Our goal is to assist community members as quickly as possible. Please bring the following items with you to the Assistance Center:

  • Driver’s license or other government-issued identification, and Social Security card
  • Proof of residency within the evacuation zone (e.g. current utility bill, etc.)
  • For minors residing in the household, proof of dependency (e.g. Social Security card)
  • Receipts for any expenses incurred while evacuated from your home
